If you're insured under a Vision Perfect or FUSION plan with an EyeMed network discount, you may choose to visit any eye care provider. However, if you visit an EyeMed network provider you may receive eye wear and services at reduced costs. Compare your ID card to the one pictured below to verify that you are enrolled in either of these plans. If so, select the directory link below and the Access network to find an EyeMed network provider.

Prior to making an appointment, and again at the time of the appointment, be sure to verify that the provider is a network provider.
